Although the position is remote, you will be connected to a collaborative network of mental health professionals in our outpatient offices. As a Licensed Professional Counselor at Grow Healthcare Services, your responsibilities will include:

• Conducting comprehensive assessments to determine patient needs and establish appropriate treatment plans.
• Communicating effectively with supervisors, multidisciplinary team members, families, and other stakeholders regarding client progress and treatment updates.
• Creating and maintaining a detailed schedule of appointments, home visits, and observations to support client outcomes.
• Coordinating necessary home visits while ensuring all observations and data analyses meet program requirements.
• Monitoring the implementation and effectiveness of treatment plans, ensuring adherence to best practices.
• Participating in Interdisciplinary Team Meetings to collaborate on client care strategies.
• Documenting client progress through detailed monthly and quarterly progress notes.
• Leading training sessions to ensure staff members are proficient in the implementation of treatment plans and documentation requirements.
• Providing ongoing follow-up support through additional consultations and home visits to ensure continuity of care.
• Maintaining accurate records in compliance with legal and ethical standards, readily available for review by authorized parties.
• Serving as a liaison between Grow Healthcare Services and the broader mental health community, contributing to program development and community engagement initiatives.

To be considered for this position, you must possess the following qualifications:

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.
• An active and unrestricted Professional Counselor (LPC) license in Tennessee.
• A minimum of two years of direct counseling experience, particularly in a clinical setting.
• A solid understanding of legal and ethical standards pertaining to mental health practice.
• Exceptional interpersonal and communication skills, capable of establishing rapport with clients from various backgrounds.
• Proven ability to work collaboratively within a healthcare team to achieve common goals.

While not mandatory, the following experiences are highly desired:

• Specialization in therapeutic modalities such as family therapy, couples therapy, trauma-informed care, or substance misuse counseling.
• Familiarity with electronic medical record (EMR) systems and related technologies for efficient patient management.
