• Conduct thorough initial assessments and ongoing evaluations of patients to develop tailored treatment plans that respond to their unique needs.
• Facilitate effective communication among supervisors, families, and other healthcare professionals to enhance patient care and track individual progress.
• Prepare and share a tentative schedule of appointments and home visits with supervisors, ensuring timely and coordinated progress monitoring.
• Coordinate and implement home visits as necessary to perform in-home observations and collect essential data for patient care analysis.
• Ensure that program plans are executed effectively, maintaining high standards of quality and fidelity in service delivery.
• Engage actively in Interdisciplinary Team Meetings and collaborate on various initiatives that support holistic patient care.
• Regularly complete monthly and quarterly progress notes to document ongoing patient engagement and treatment outcomes.
• Organize and conduct training for staff on the implementation of program protocols and documentation practices.
• Provide follow-up support through interviews, home visits, and observations, guaranteeing continuity of care.
• Maintain accurate and easily accessible records to facilitate collaboration with external mental health professionals and community resources.
• Act as a liaison between the public and mental health sectors, promoting collaborative efforts toward developmental goals within the community.
• Assist in designing training materials and procedures to enhance program efficacy and participant experiences.

As an LPC at Congruent Health Center, you will work with a diverse client base that includes adults, youths, families, and couples. Clients may present with a wide array of mental health concerns, including but not limited to trauma, substance misuse, anxiety, and depression. Your role will involve not just offering therapeutic interventions but also cultivating a supportive environment that fosters resilience and strength.

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
• Must possess active and unrestricted licensure as a Professional Counselor (LPC) within the State of Texas.
• A minimum of two years of post-degree clinical experience in counseling is essential.
• Specialization in areas such as family therapy, trauma-informed care, couples counseling, or substance misuse is preferred.
• Strong understanding of legal and ethical standards related to mental health practice.
• Outstanding communication and interpersonal skills, enabling effective rapport-building with varied clientele.
• Ability to collaborate effectively within a multidisciplinary healthcare team.
• Familiarity with electronic medical records (EMR) software and other digital tools used in mental health documentation.

• Experience in community health settings or similar environments is a plus.
• Knowledge of trauma-informed care practices and techniques.
• Demonstrable skills in developing treatment plans and delivering evidence-based interventions.
