• Conduct in-depth clinical assessments to evaluate the mental health needs and concerns of patients and families.
• Develop individualized treatment plans that incorporate evidence-based practices tailored to the unique circumstances of hospice patients.
• Employ therapeutic modalities such as cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) in one-on-one and family sessions, adapting your techniques as needed based on patient responses.
• Document all clinical interactions and treatment progress in a thorough and timely manner, ensuring compliance with HIPAA and organizational policies.
• Collaborate with interdisciplinary team members, including physicians and grief counselors, to provide coordinated care that addresses both physical and emotional needs.
• Participate in peer reviews and case consultations, sharing insights and integrating feedback to enhance care strategies.
• Provide psychoeducation and support to patients and their families, empowering them with resources to navigate the complexities of hospice care.
• Stay current with mental health best practices, incorporating new methodologies and research into clinical work.

In this role, you will primarily work with terminally ill patients and their families, who often experience a wide range of emotional challenges, including grief, anxiety, and depression. This unique patient population seeks not only to manage symptoms but also to find peace and closure as they approach the end of life. You will play a critical role in fostering a safe space for open dialogue and emotional support, aiding family members as they cope with impending loss.

• A Master's degree in Social Work from an accredited institution.
• A valid state license as a Licensed Clinical Social Worker (LCSW).
• A minimum of 3 years of experience providing psychotherapy in a clinical setting, particularly with hospice or palliative care experience preferred.
• Strong knowledge of solution-focused therapeutic interventions and techniques.
• Exceptional communication and interpersonal skills, capable of building rapport with patients and families.
• Demonstrated critical thinking and problem-solving abilities.
• A collaborative approach to working within multidisciplinary teams.

• Familiarity with hospice care and understanding of the emotional and psychological needs surrounding end-of-life issues.
• Experience with grief counseling and bereavement support.
• Proficiency in using electronic health record (EHR) systems for documentation.
