Our hospice center operates in a remote setting, allowing for flexible scheduling tailored to meet the needs of our clients. You will primarily work from home but maintain regular communication with other team members, including psychiatrists, psychologists, and medical staff. This role not only adapts to the unique needs of each client but also allows for necessary evening and weekend hours, reflecting our commitment to providing timely and effective care.

• Conduct comprehensive psychosocial assessments to identify clients' mental health concerns, strengths, and resources specifically related to hospice and palliative care.
• Provide individual, family, and group therapy utilizing evidence-based treatment modalities tailored for end-of-life issues, such as cognitive-behavioral therapy, solution-focused therapy, and trauma-focused approaches.
• Develop and implement individualized treatment plans that are sensitive to the emotional, spiritual, and medical needs of clients and their families.
• Facilitate group therapy sessions and provide psychoeducation to clients on coping strategies, grief management, and family dynamics during the hospice process.
• Engage in crisis intervention, conducting risk assessments, and devising safety plans as required.
• Maintain accurate and timely documentation that adheres to ethical and legal standards to ensure continuity of care.
• Collaborate effectively with multidisciplinary teams to create comprehensive care plans that reflect client-centered approaches.

This role primarily focuses on individuals receiving hospice care and their families, catering to a diverse clientele with unique emotional and psychological needs. You will support clients facing severe illnesses, manage grief and loss, and enhance overall well-being during a challenging life phase. Your work will significantly contribute to their ability to cope and find comfort during end-of-life transitions.

• Master's degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong foundation in evidence-based therapeutic modalities applicable in hospice settings.
• Excellent interpersonal, communication, and assessment skills to effectively engage with clients and families.

• A minimum of two years of experience in a mental health or outpatient setting, preferably with a focus on hospice or palliative care.
• Experience in facilitating group therapy sessions and conducting family therapy in a sensitive manner.
• Familiarity with crisis intervention techniques and risk assessment protocols.
