Key Responsibilities

As a Licensed Clinical Social Worker, you will be responsible for a range of essential tasks aimed at improving the mental health of your clients. Your daily responsibilities will include:

  • Conducting thorough, age and culturally appropriate biopsychosocial assessments for individuals with mental health, substance abuse, and medical needs in a primary care setting.
  • Providing a wide range of behavioral health services, which encompass individual and group counseling, intake assessments, diagnostic evaluations, treatment planning, and crisis intervention.
  • Facilitating referrals to community resources and agencies, including medication assistance programs, to ensure clients receive comprehensive support during their treatment.
  • Documenting client interactions meticulously through electronic health records, including mental health evaluations, progress notes, and suicide risk assessments, ensuring accurate and timely information is available to the care team.
  • Collaborating with Quality Improvement teams to enhance processes for data collection and analysis, fostering a culture of continual enhancement in service delivery.
Patient Population / Client Focus

The role primarily involves working with individuals and families facing significant mental health challenges, including those in hospice care. You will engage with an increasingly diverse patient population, addressing a wide range of emotional and psychological needs, helping guide them through their unique journeys with empathy and expertise.

Qualifications

To be eligible for this role, candidates must meet the following qualifications:

  • Current state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A Masterโ€™s degree in Social Work, complemented by at least three years of professional experience post-licensure, is required.
  • Proficiency in counseling techniques, crisis intervention strategies, and the capability to exercise independent judgment in clinical situations is critical.
Preferred Experience

While not mandatory, preferred experience includes:

  • Demonstrated skills in group facilitation and planning processes.
  • A track record of effectively developing treatment plans and conducting diagnostic evaluations.
