You will engage with clients mainly via telehealth platforms, telephone consultations, and in-home visits when necessary. • Conduct thorough assessments of seniors' psychosocial needs, taking into account their emotional, mental, and social challenges.
• Develop personalized care plans in collaboration with clients and their families, aligning interventions with their specific goals and preferences.
• Provide counseling and emotional support, assisting clients in navigating critical life transitions and overcoming challenges.
• Advocate for clients’ rights and facilitate access to essential community resources, enabling them to receive appropriate services and support.
• Regularly monitor clients’ well-being through scheduled assessments and home visits, ensuring their safety and addressing emerging concerns.
• Education of clients and families about community resources and support groups to promote social engagement and personal empowerment.
• Maintain detailed and accurate documentation of client interactions, assessments, and progress within the electronic health record (EHR) system.
