This position operates in a remote setting, providing the flexibility to manage your own schedule while ensuring that our patients receive the attention and care they require. • Perform comprehensive mental health assessments, including in-depth psychosocial evaluations and risk assessments to identify the unique needs of each patient.
• Work closely with patients and their families to develop tailored treatment plans that incorporate individual goals, cultural backgrounds, and specific mental health challenges.
• Utilize evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) in various settings, including individual, couple, family, and group sessions.
• Provide critical crisis intervention services and collaborate with other healthcare professionals to ensure continuity of care for patients experiencing acute emotional distress.
• Maintain accurate and timely clinical documentation, including progress notes, treatment plans, and discharge summaries, following clinic policies and regulatory guidelines.
• Empower patients and their families through psychoeducation, equipping them with resources and strategies to manage mental health conditions effectively.
• Engage in continuous collaboration with a multidisciplinary team, fostering an environment of shared knowledge to enhance patient care.
• Regularly conduct outcome assessments to evaluate the effectiveness of treatment interventions, making necessary adjustments to optimize care.
• Stay current with developments in mental health practices and research, implementing new insights into your clinical practice to improve patient outcomes.
• Adhere strictly to ethical guidelines, protecting patient confidentiality at all times.

Your primary focus will be on patients within the hospice setting, who often face complex emotional and psychological challenges as they confront serious health issues or terminal diagnoses. You will work with a diverse population, providing culturally competent care that respects individual backgrounds and addresses specific mental health needs.

• A Master’s degree in Social Work (MSW) is required, along with an active state license as a Licensed Clinical Social Worker (LCSW).
• Demonstrated ability to work autonomously, as well as collaboratively within a multidisciplinary healthcare team.
• Exceptional interpersonal and communication skills, with a proven track record of establishing rapport with clients from various cultural backgrounds.
• Strong organizational skills and the ability to manage multiple cases effectively while maintaining high standards of care.

• A minimum of two years of clinical experience in a mental health or hospice setting, demonstrating a deep understanding of end-of-life issues and interventions.
• Experience in delivering a range of therapeutic modalities tailored to the unique needs of hospice patients and their families.
• Familiarity with electronic health record (EHR) systems and strong documentation skills.
