As a Licensed Clinical Social Worker with Gotham Enterprises Ltd, your responsibilities will include:

• Conducting thorough clinical assessments to understand the psychological and social factors affecting patients and their families.
• Developing individualized treatment plans grounded in evidence-based practices to address mental health and emotional needs.
• Implementing various therapeutic interventions, including cognitive-behavioral therapy (CBT) and narrative therapy, adapting treatment modalities based on patient circumstances.
• Maintaining detailed and accurate clinical documentation that complies with HIPAA regulations while ensuring punctuality in record-keeping.
• Collaborating closely with psychiatrists, nurses, and other healthcare professionals to create a cohesive treatment strategy for patients.
• Participating in case consultations and peer review meetings to continually improve the standard of care provided.
• Educating patients and their families about mental health concerns, treatment pathways, and community resources that can support their journey.
• Staying informed on the latest research and advancements in social work and mental health to ensure best practices in patient care.

Your role as an LCSW will focus on hospice patients, primarily those facing terminal illnesses, providing critical support to both the patients and their families. You will handle emotional distress associated with grief, loss, and end-of-life issues, necessitating a profound level of compassion and understanding. Your interventions will help patients achieve dignity and peace, with an emphasis on improving quality of life in their final months.

The ideal candidate must possess the following qualifications:

• A Master’s degree in Social Work from an accredited university.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state of Arizona.
• At least three years of clinical experience in a healthcare setting, preferably in hospice or palliative care.
• Demonstrated expertise in using solution-focused therapeutic interventions.
• Exceptional interpersonal and communication skills for effective collaboration with patients, families, and team members.

• Experience working with patients facing end-of-life issues or terminal ailments is highly desirable.
• Familiarity with interdisciplinary approaches within hospice care settings.
• Previous experience with bereavement counseling is a plus.
