Work Environment

This position is entirely remote, allowing you the flexibility to provide care from the comfort of your home while maintaining necessary oversight and collaboration with our healthcare team. All appointments will be scheduled based on client needs and therapist availability.

Key Responsibilities
  • Collaborate with primary care providers to identify and manage mental health and psychosocial issues within the patient population.
  • Conduct comprehensive patient intakes, focusing on diagnostic and functional evaluations, followed by tailored recommendations to the healthcare team regarding treatment goals and plans.
  • Provide brief, focused mental health interventions via telehealth to fit the specific requirements of patients needing support.
  • Educate patients and provide self-management strategies empowering them to participate actively in their treatment and recovery.
  • Monitor patient progress through regular follow-ups, recognizing the need for treatment adjustments to effectively promote mental health stability.
  • Document each patient's progress and maintain accurate treatment records, complying with the standards set forth by the Agency and Federally Qualified Health Center (FQHC).
  • Facilitate communication with community resources and engage in plans to prevent worsening mental health conditions.
  • Assist patients in managing any medical treatments prescribed by primary care providers, ensuring effective coping strategies are established for treatment side effects.
Patient Population / Client Focus

You will work with a diverse range of clients, including children, adolescents, adults, and families, who are dealing with various mental health challenges. Understanding the unique personal and social impacts of various psychosocial issues on individuals and their families is vital to your role as a counselor. Furthermore, providing bilingual support in English and Spanish is highly valued, creating a more inclusive environment for our patients.

Qualifications
  • A Masterโ€™s Degree in Clinical Counseling from an accredited institution.
