The Guided Behavioral Reflection Clinic is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our compassionate team. Work Environment

This role offers a fully remote working arrangement, providing flexibility to manage your professional responsibilities from any location. The position is part of a hospice center team, where compassion and support are at the forefront of our interactions. While the standard work hours are Monday through Friday from 8:00 AM to 4:30 PM, occasional evening and weekend hours may be necessary to meet client needs. On-call availability may also be required to provide urgent support during off-hours, ensuring continuous care for our clients.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments of senior clients, considering emotional, social, and environmental factors affecting their well-being.
  • Collaborate with clients and their families to develop tailored care plans that align with their individual goals, preferences, and needs.
  • Provide counseling and emotional support to seniors during life transitions, offering guidance and strategies to navigate challenges.
  • Advocate for clients' rights and facilitate access to essential community resources, ensuring they receive appropriate and timely services.
  • Work collaboratively with interdisciplinary teams, including nurses, caregivers, and other healthcare professionals, to coordinate integrated care efforts.
  • Perform regular visits to assess clients' living conditions, monitor well-being, and address any concerns or safety issues.
  • Facilitate effective communication and mediate conflicts among family members involved in the care of senior clients.
  • Educate clients and families on available resources, support groups, and programs to enhance their social engagement and overall quality of life.
  • Maintain rigorous documentation of client assessments, care plans, and progress notes to ensure compliance with regulatory standards.
  • Stay updated on current social work practices, policies, and ethical guidelines to provide the highest quality of care.
Patient Population / Client Focus

Qualifications

Compensation and Benefits

The salary for this position ranges from USD 70,000 to 100,000 annually, commensurate with experience and qualifications.
