What is a remote forms designer?

Remote Forms Designers are professionals who create, customize, and optimize digital forms for organizations, such as surveys, registration forms, or feedback forms, while working from a remote location. They use various form-building software and tools to design user-friendly and visually appealing forms that meet the business's specific needs. Their role often includes collaborating with clients or teams to ensure forms collect the right data efficiently and securely. Remote Forms Designers need strong attention to detail, design skills, and familiarity with online form platforms.

What skills and qualifications are needed to thrive as a remote forms designer?

Familiarity with form building platforms (such as Jotform, Typeform, or Adobe Acrobat), HTML/CSS, and workflow automation tools is typically required. Strong attention to detail, problem-solving abilities, and effective communication skills help you collaborate remotely and tailor forms to client needs. Job description

Position Overview

Foothill Mental Healthcare Pte. Ltd is seeking a dedicated and compassionate Licensed Mental Health Counselor to join our dynamic team, operating in a convenient remote capacity from Chandler, Arizona. As a key member of our mental health team, you will provide essential therapeutic support to individuals and families in need of mental health services via telehealth, contributing significantly to patient well-being and recovery.

About the Organization

At Foothill Mental Healthcare, we are dedicated to delivering person-centered, holistic care that integrates the latest evidence-based advances in neuroscience. Our mission centers on making high-quality mental health care accessible to everyone. We value a diverse and inclusive workplace and pride ourselves on a supportive and collaborative environment where mental health professionals are empowered to excel and innovate.

Work Environment

This position is fully remote, offering flexibility and the opportunity to work from home while still engaging in meaningful therapeutic practice. Our telehealth platform is designed to provide seamless interactions with clients, fostering therapeutic relationships and effective counseling sessions without the constraints of a traditional clinic setting.

Key Responsibilities
  • Conduct culturally sensitive and strength-based assessments for new patients and their families, identifying their unique needs and circumstances.
  • Provide both individual and family therapy tailored to the assessed needs, utilizing established therapeutic modalities and techniques.
  • Ensure timely completion of billing, documentation, individualized treatment plans, and insurance authorization forms, adhering to organizational deadlines.
  • Develop comprehensive crisis plans, safety plans, relapse prevention strategies, and wellness plans that promote stable functioning and resilience in clients.
  • Create detailed discharge plans that support clients moving towards independence and further care.
  • Strictly adhere to all pertinent federal and state regulations, as well as internal policies and procedures of Foothill Mental Healthcare.
  • Engage in continuous professional development through training and continuing education to maintain and enhance clinical skills.
  • Participate actively in case consultations and agency meetings to foster collaboration and share insights among peers.
  • Complete all required licensure and mandatory training to ensure compliance and quality service delivery.
Patient Population / Client Focus

In this role, you will work with a diverse array of clients, including children, adolescents, and adults. Each case presents unique challenges and opportunities to make a significant impact on the lives of individuals experiencing mental health issues, from anxiety and depression to more complex conditions. Your role will be instrumental in reducing stigma associated with seeking help and improving access to mental health resources.

Qualifications
  • A Masterโ€™s degree in social work, counseling, marriage and family counseling, or a related field is required, which qualifies you for licensure.
  • Current licensure as a Licensed Mental Health Counselor (LPC, LMFT, LCSW) in the State of Arizona is mandatory.
Preferred Experience

While not required, experience in telehealth services, familiarity with client management software, and training in evidence-based treatment methodologies can enhance your candidacy. We prefer candidates who have a demonstrated ability to build rapport with clients remotely and who possess strong skills in cultural competency.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 80,000 to 100,000, commensurate with your licensure and experience. Our comprehensive benefits package includes:

  • Flexible work schedules with options for full-time and part-time roles, promoting a healthy work/life balance.
  • Generous paid time off (PTO) policy, allowing you to recharge and maintain your personal well-being.
  • Health and dental insurance to ensure you and your family have access to quality healthcare.
  • Short- and long-term disability insurance, providing peace of mind in unforeseen circumstances.
  • Life insurance coverage to protect your loved ones.
Career Growth Opportunities

Foothill Mental Healthcare is committed to the professional development of its team members. As part of our organization, you will have access to ongoing training and mentorship programs, enabling you to expand your clinical skills and pursue advanced certifications or specializations in your field.

Why Join Our Team

Joining Foothill Mental Healthcare means becoming part of a progressive team dedicated to transforming lives through compassionate care. You will be supported in your professional journey and help us realize our mission of increasing access to mental health services while reducing stigma. Together, we can create a meaningful impact in the communities we serve.
