Job description

We are looking for a dedicated Registered Nurse to join our team in the Bronx. Help New York residents achieve a better quality of living by conducting UAS assessments and providing leadership to our regional team of Nurse Evaluators.

The Basics
  • Pay Rate: $60/hr

  • Location: Bronx, NY (3 days in community , 2 days remote)

  • Hours: Monโ€“Fri (8amโ€“6pm) | Satโ€“Sun (10amโ€“6pm) Schedule flexibility available. 37.5 hours per week.


Key Responsibilities
  • Field Evaluations: Perform in-home patient evaluations in the Bronx area focusing on health strengths and care preferences via the UAS-NY tool.

  • Team Support: Act as a lead by instructing and observing new Nurse Evaluators during assessment visits to ensure high-quality service.

  • Administrative Oversight: Review consumer health referral forms and documentation for accuracy; transmit data into electronic forms promptly.

  • Community Impact: Facilitate enrollment into managed care health plans to ensure patients receive the support they need.

Qualifications
  • Licensure: Valid RN license in the state of New York.

  • Experience: 2+ years of RN experience in home care.

  • Technical Skills: Prior experience using the UAS-NY Community assessment tool.

  • Travel: Ability to travel locally within the Bronx and surrounding area up to 50%.

  • Home Office: Reliable high-speed internet (min 20 Mbps download) and a personal cell phone.
