• Conduct thorough psychosocial assessments to gather detailed information regarding clients’ mental health conditions, strengths, and resources.
• Provide tailored therapeutic interventions in individual, family, and group settings using evidence-based approaches such as cognitive-behavioral therapy and trauma-focused therapy.
• Develop and implement individualized treatment plans that are responsive to the specific needs and goals of each client.
• Collaborate effectively with multidisciplinary team members to ensure seamless coordination of care and holistic treatment strategies.
• Facilitate psychoeducation sessions and support groups to enhance client understanding of mental health and coping strategies.
• Offer crisis intervention and risk assessments, developing safety plans as necessary to provide immediate support.
• Maintain meticulous documentation in adherence to ethical and legal standards, ensuring all client interactions and treatment plans are recorded accurately.
• Lead group therapy sessions that foster a sense of community among participants and provide shared opportunities for healing.

Our clientele encompasses a diverse group of individuals facing a variety of mental health issues, particularly those linked to end-of-life care and chronic illness, as we work closely with patients in hospice settings. This role demands a compassionate approach, recognizing the emotional complexities involved in delivering mental health support to individuals and families during vulnerable times.

• Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Independent Clinical Social Worker (LICSW).
• A minimum of two years’ experience in a mental health or outpatient care environment, with a focus on therapeutic practices.
• Strong understanding of evidence-based treatment modalities and theoretical approaches to therapy.
• Exceptional interpersonal, communication, and assessment skills, allowing for effective client engagement and rapport building.

• Prior experience in hospice, end-of-life care, or crisis intervention settings is strongly preferred.
• Familiarity with mental health technologies and teletherapy practices is advantageous.
• Participation in ongoing education and training programs related to advanced therapeutic practices.
