• Perform comprehensive mental health assessments and screenings to determine the individual needs of patients, establishing effective treatment plans tailored to their unique situations.
• Provide therapeutic interventions, including individual counseling and crisis management for patients experiencing a spectrum of mental health issues, such as anxiety, depression, trauma, and substance abuse.
• Facilitate group therapy sessions and workshops focused on personal development, coping strategies, and relevant therapeutic themes to foster a supportive community among patients.
• Collaborate closely with healthcare providers, community organizations, and support teams to ensure a holistic approach to patient care, making referrals as necessary to address physical or social needs.
• Participate in community outreach programs aimed at increasing mental health awareness and prevention, contributing to initiatives that promote wellness and reduce stigma.
• Maintain thorough and confidential documentation of patient progress and treatment plans, adhering to all ethical guidelines and legal requirements.

The position primarily focuses on serving a diverse patient population within the community health clinic setting. This includes individuals from various socioeconomic backgrounds, ages, and cultures who may present unique and complex mental health challenges. • A valid license as a Mental Health Counselor in Illinois, such as Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).
• Demonstrated educational background with a minimum of a Bachelor's degree in Counseling, Psychology, Social Work, or a related field; Master’s or doctoral degree preferred.
• Strong understanding of mental health issues specific to community health settings, evidenced by prior experience in similar environments.
• Exceptional communication and interpersonal skills, enabling effective and empathetic engagement with patients and collaboration with a multidisciplinary healthcare team.
• Proven knowledge of evidence-based practices and therapeutic models appropriate for diverse patient needs, along with a flexible approach to therapy.

• A minimum of 2 years of clinical experience in a mental health counseling role, preferably within a community health or clinical setting.
• Experience in conducting group therapy and community outreach programs, demonstrating the ability to inspire and support client engagement and participation.
• Familiarity with telehealth platforms and digital health tools to facilitate remote therapy sessions is highly desirable.
