• Conduct comprehensive assessments to identify patients’ needs and formulate personalized treatment plans that promote mental well-being.

You will work primarily with patients in hospice care, specializing in supporting individuals dealing with end-of-life transitions. Your role is crucial in providing both mental health support and critical emotional resources to patients and their families, ensuring they receive compassionate care during a sensitive period.

• A Master’s degree in Social Work from an accredited institution is required to assess and treat patients effectively.
• A valid LCSW license in the state of Illinois is mandatory to practice in this setting.
• At least 2 years of clinical experience is preferred, particularly with adult populations or hospice settings.
• Proficiency in solution-focused psychotherapy methods enhances your ability to provide effective interventions.
• Strong communication and interpersonal skills will help foster positive relationships with clients and team members.
