This position allows for a rewarding remote work setting, enabling the Social Worker to engage with clients from the comfort of their own homes. Our work hours are typically Monday to Friday from 8:00 AM to 4:30 PM, although flexibility is needed to accommodate evening or weekend visits based on client availability. The successful candidate will be an integral part of a cohesive, interdisciplinary team that includes nurses, healthcare aides, and medical staff, ensuring a comprehensive approach to care.

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting thorough psychosocial assessments to identify the emotional, social, and psychological needs of clients and their families.
• Developing tailored care plans that incorporate client and family goals, ensuring each plan is suitable for varying situations in hospice care.
• Providing one-on-one counseling to help clients navigate complex emotions and life transitions, offering support during their end-of-life journey.
• Actively advocating for clients’ access to community resources, including support groups, respite care, and financial resources, thereby enhancing their overall quality of life.
• Monitoring clients' well-being through regular check-ins, evaluating their living conditions, and addressing any safety or health concerns.
• Facilitating communication among family members to resolve conflicts and ensure cohesive caregiving, allowing for a supportive family dynamic during end-of-life care.
• Documenting all client interactions, updates, and evaluations meticulously, maintaining compliance with regulatory standards, and utilizing electronic health record systems effectively.
• Staying apprised of current policies and best practices in social work to uphold the highest ethical standards in care delivery.

• A Bachelor’s degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience working as a Social Worker, preferably in a hospice, geriatric, or community center setting.
• Deep understanding of social work principles and best practices, along with familiarity with aging-related issues and community resources.
• Strong assessment, counseling, and communication skills, with the ability to establish rapport and trust quickly.
• Proven ability to work effectively with interdisciplinary teams, showing flexibility and a collaborative spirit.
• Computer proficiency and experience with electronic health record systems are highly advantageous.
