Key Responsibilities

  • Collaborate closely with multidisciplinary healthcare teams to devise and implement personalized treatment plans, including thorough discharge planning based on psychosocial assessments and accepted social work methodologies.
  • Conduct counseling sessions and provide crisis intervention services to patients, utilizing evidence-based techniques to assess and treat emotional and behavioral challenges.
  • Assist clients in their discharge planning and care coordination, connecting them with regional resources and facilitating access to essential services that support their recovery journey.
  • Consult with family members and support systems regarding client treatment options and strategies for successful discharge planning, ensuring a holistic support network for patients.
Patient Population / Client Focus

The ideal candidate will work with a diverse client population experiencing various mental health challenges, including anxiety, depression, trauma, and behavioral issues. Qualifications
  • A Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology is required.
  • Current licensure as a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW), or equivalent credential.
  • Minimum of three years of experience working in an outpatient mental health setting.
  • Demonstrated expertise in delivering evidence-based therapy and mental health services tailored to the needs of patients.
  • Strong organizational skills and an ability to work independently in a fast-paced environment.
  • Proficient in using technology, including MS Office and various telehealth platforms, essential for effective documentation and patient interaction.
