This position is remote, enabling you to provide crucial support from the comfort of your own home while connecting with patients and colleagues through a robust digital infrastructure. As a remote social worker at a hospice center, you will collaborate with other healthcare professionals, including nurses and support staff, maintaining a multidisciplinary perspective. • Conduct thorough biopsychosocial evaluations to identify and eliminate barriers to treatment, enhancing patients' health status and improving resource utilization.
• Develop and implement comprehensive treatment plans utilizing both individual and group therapy modalities, ensuring continuous assessment and adjustment to maximize patient outcomes.
• Provide education and counseling to patients and their families, helping them understand and accept medical and social recommendations while fostering their ability to navigate challenges effectively.
• Collaborate with the multidisciplinary team by providing insightful consultations regarding patients' psychosocial functioning and needs, facilitating improved treatment compliance and care coordination.
• Document all social work activities meticulously in patients' medical records, adhering to departmental standards and legal regulations.
• Remain updated on community resources and advocacy opportunities, helping patients and families access necessary support while emphasizing self-determination.
• Engage in continuous professional development through relevant training and educational activities to enhance expertise and clinical skills.
• Participate in case-related statistics and departmental reporting to ensure compliance and support organizational goals effectively.

This position focuses on assisting patients within a hospice care setting, where the emphasis lies on supporting individuals facing end-of-life issues and their families. You will work with a diverse group of patients, considering their unique psychosocial needs and cultural backgrounds. Your role will be fundamental in providing emotional support and facilitating the acceptance of the end-of-life journey.

• A Master’s Degree in Social Work from an accredited graduate program.
• A current Licensed Clinical Social Worker (LCSW) certification from the State Board of Social Work Licensure.
• Demonstrated knowledge and skills necessary to effectively address the needs of patients considering their developmental stages and cultural contexts.

• A minimum of one year of clinical experience, preferably in hospice, palliative care, or mental health settings.
• Experience providing care to varied populations, with a focus on geriatric clients and families dealing with grief and loss.
• Familiarity with community resources available for patients and families in hospice contexts, enhancing support initiation and navigation.
