• Collaborate with the Social Work Manager to assess, diagnose, and treat a variety of psychosocial issues impacting patients and their families, aiming to promote optimal social functioning.
• Uphold the ethical standards of the NASW Code of Ethics while delivering compassionate and comprehensive mental health therapy to patients.
• Conduct thorough bio-psychosocial evaluations to identify and address barriers to treatment and improve resource utilization, ensuring a seamless transition between care pathways.
• Engage with medical and support staff to offer critical insights regarding the psychosocial needs of patients, contributing to the formulation of individualized care plans.
• Design and implement intervention strategies, utilizing both individual and group therapy modalities tailored to patient needs.
• Document all social work activity in patients' medical records in compliance with departmental standards, adhering to frequency and content guidelines.
• Educate patients and their families about recommended medical and social work strategies aimed at facilitating recovery and restoring functionality.
• Identify advocacy needs and coordinate community resources to enhance social support and client empowerment.
• Maintain statistical records for case management and ensure compliance with the highest standards of care and legal regulations, contributing to a continuous improvement culture.
• Engage in ongoing professional development through training and educational activities, ensuring expertise in the latest treatment methodologies and community resources.

Your role as an LCSW will focus on supporting individuals in a hospice setting who are often facing life-limiting illnesses or significant emotional distress. This position requires a deep understanding of the psychological and social challenges faced by patients and families during this delicate time. The ideal candidate will demonstrate empathy and a keen ability to connect with individuals from diverse backgrounds and cultures, ensuring that every patient receives dignified, tailored support.

• A Master’s Degree in Social Work from an accredited institution.
• A current Licensed Clinical Social Work (LCSW) license issued by the State Board of Social Work Licensure is mandatory.
• Well-rounded knowledge and skills specific to the multidisciplinary treatment of patients, incorporating consideration of aging processes and cultural variations in each interaction.
• A minimum of one year of clinical experience, ideally within a healthcare or mental health setting, focusing on populations similar to those served in hospice care.
• Exceptional analytical and therapy skills, with the ability to create effective treatment plans and evaluate therapy outcomes.

• Prior experience working with hospice patients or in end-of-life care settings is highly preferred.
• Familiarity with community resources and support services specific to hospice and end-of-life care.
• Experience in collaborating effectively in a multidisciplinary team while maintaining strong communication skills in virtual settings.
