Role Description

At Atlas Oncology, we believe people facing cancer deserve not only medical excellence but compassionate, coordinated support. We are seeking a dedicated Registered Nurse to join our interdisciplinary care team and serve as a clinical anchor for patients navigating the cancer care continuum. In this role, you will ensure that patients receive timely, proactive, and holistic care by facilitating communication, addressing barriers, and coordinating services across oncology, behavioral health, and supportive care.

As a consistent point of contact, you will provide education, advocacy, and personalized support that empowers patients and enhances quality of life—while partnering with a team that values innovation, empathy, and whole-person care.

Responsibilities include, but not limited to, the following:

· Serve as a consistent clinical liaison between patients, families, and the oncology care team

· Conduct comprehensive assessments and develop individualized care plans tailored to each patient’s medical, psychosocial, and behavioral health needs

· Provide education to patients and families regarding diagnosis, treatment, symptom management, and survivorship

· Coordinate care across disciplines, schedule appointments, and facilitate timely follow-up

· Identify and address barriers to care, including financial, transportation, or cultural challenges

· Deliver proactive outreach, regular check-ins, and telephone triage to ensure continuity and early issue identification

· Document care coordination activities and patient interactions in the EMR clearly and accurately

· Participate in interdisciplinary team meetings and contribute to shared care planning and quality improvement

· Collect and document patient history and current symptoms.

· Evaluate and prioritize patient symptoms and concerns based on urgency and severity.

· Recognize urgent and emergent situations requiring care escalations.

· Communicate with supportive care staff (navigators, social workers) to ensure the patient has access to appropriate support services, or additional services as needed.

· Support shared decision-making, patient empowerment, and health equity through culturally responsive care

Required Qualifications

· Active RN license

· Minimum of 3 years of RN experience in a healthcare setting

· Excellent communication, documentation, and interpersonal skills

· Ability to navigate sensitive clinical situations with compassion and professionalism

· Comfort with change, process improvement, and working in a start-up or growing organization

Preferred Qualifications

· Bachelor’s degree in nursing (BSN) preferred

· Certification in Oncology Nursing (OCN) or related specialty preferred

· Value based primary care experience

· 2-3 years’ experience in oncology treatment and symptom management

· Experience working with diverse populations and an understanding of health disparities

· Experience with athenahealth
