• Conduct detailed mental health assessments that take into account the intersection of psychiatric and physical health issues affecting patients.
• Design and implement individualized treatment plans that address both primary mental health concerns and any co-occurring medical conditions.
• Facilitate individual and group therapy sessions, utilizing evidence-based methodologies adapted to each patient’s unique circumstances.
• Work collaboratively with medical providers to ensure seamless care coordination and optimal treatment outcomes.
• Provide crisis intervention and emergency support, responding quickly to mental health crises to ensure patient safety.
• Educate patients on the link between mental health and physical wellness, equipping them with the tools to take charge of their health.
• Maintain precise, confidential documentation in line with legal and ethical standards, keeping accurate records of patient interactions and progress.
• Engage actively in team meetings, case conferences, and interdisciplinary planning sessions to foster collaborative decision-making.
• Stay informed about the latest research, best practices, and innovations in the field of mental health and integrated care.

Your patient population will comprise individuals grappling with various mental health challenges, often coupled with complex medical conditions. These may include autoimmune disorders, chronic pain, and substance use disorders. We aim to support our patients holistically, prioritizing their overall well-being and recovery through an integrated care framework.

• A Bachelor's degree in Social Work, Mental Health Counseling, Psychology, or a related field.
• Current licensure as a Mental Health Counselor (LMHC), Licensed Clinical Social Worker (LCSW), Licensed Psychoanalyst, Psychologist, or Licensed Marriage and Family Therapist (LMFT) in New York.
• A deep understanding of mental health issues and the impact of co-occurring medical conditions.

• Experience working with diverse populations and understanding the nuances of providing care to individuals with complex needs.
• Familiarity with electronic health record systems and telehealth platforms to facilitate effective patient management.
• Strong interpersonal skills that promote open communication and trust with patients and interdisciplinary team members.
