This role is designed for remote work, offering flexibility and the comfort of working from home. However, as a social worker embedded in a Hospice Center, you will have the unique opportunity to connect with clients and their families virtually or in the community. • Engage directly with clients through telehealth platforms, providing therapeutic interventions to those battling symptoms of severe mental health issues.
• Collaborate with healthcare professionals to design and implement integrated care plans tailored to individual client needs.
• Conduct evidence-based screenings and assessments, utilizing various therapeutic modalities to determine treatment approaches.
• Facilitate effective communication among clients, their families, and service providers to ensure continuity of care.
• Assist clients in navigating community resources and support services, thereby improving their overall wellness.
• Maintain ongoing documentation of client progress and services in compliance with ethical guidelines and legal standards.

• A Master’s degree in Social Work from an accredited institution.
• Must possess a valid Licensed Independent Clinical Social Worker (LICSW) credential.

• Experience working with clients in hospice, palliative, or related settings.

We offer a competitive salary ranging from $70,000 to $85,000 annually, commensurate with experience and qualifications.
