• Conduct comprehensive assessments and evaluations to understand clients' mental health needs and develop tailored treatment plans.
• Maintain clear communication with supervisors, direct care staff, and family members regarding clients' progress, ensuring a collaborative treatment experience.
• Coordinate and schedule home visits as needed to observe and support clients in their natural environments.
• Participate actively in interdisciplinary team meetings to contribute insights on treatment strategies and support the overall goals of the organization.
• Complete accurate and timely progress notes on a monthly and quarterly basis to document clients' advancements and changes in treatment plans.
• Organize training sessions for fellow staff and team members to ensure all involved are well-equipped to implement program plans and documentation effectively.
• Act as a liaison with the wider mental health community, enhancing collaboration and resource development initiatives.
• Guide follow-up care, conducting additional home visits and interviews to provide ongoing support based on clients' evolving needs.
• Maintain organized and confidential records to ensure accessibility of information for relevant parties.
• Assist in the development and refinement of training materials aimed at enhancing program effectiveness and staff competencies.
