Position Overview

Intimate Bridge Counseling is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team in a full-time capacity. This remote position is focused on delivering high-quality emotional and developmental support to children and families participating in hospice services. The LCSW will play a critical role in enhancing service delivery through comprehensive assessments, individualized planning, and family support.

Work Environment

This role operates within a hospice setting and offers the flexibility of remote work. As part of a dedicated team, you will collaborate with fellow professionals to provide holistic care to families with children facing significant health challenges. Key Responsibilities
  • Conduct detailed evaluations and assessments to determine the needs of children and families engaged in hospice services.
  • Actively participate in the development and implementation of Individualized Family Service Plans (IFSP), tailored to meet the unique needs of each child.
  • Monitor outcomes regularly, adjusting and redefining strategies in collaboration with a multidisciplinary team while establishing a network of support for families.
  • Deliver educational and developmental support that empowers families, leveraging evidence-based practices and natural learning environment methodologies.
  • Establish positive rapport with children and their families, promoting an interactive and nurturing atmosphere conducive to learning and growth.
  • Ensure compliance with all federal and state regulations governing social work practices and ensure the safety of both staff and clients.
  • Utilize various teaching methods to accommodate individual learning styles and promote successful outcomes in child development.
Patient Population / Client Focus

The focus of this role is on young children with disabilities or developmental delays within the hospice care framework. The LCSW will support families in navigating the complexities of hospice services, offering emotional counsel and guidance to enhance their caregiving skills while advocating for the childโ€™s development and well-being.

Qualifications
  • A Master's Degree or Doctorate in Social Work from an accredited institution.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) by the appropriate state body.
  • At least one year of direct experience working with children in early intervention settings.
  • Strong understanding and commitment to Natural Learning Environment Practices.
  • A valid driverโ€™s license is essential for transportation to family meetings and community resources.
