Key Responsibilities

In your role as an LCSW, you will engage in a variety of tasks that directly impact patient care, including:

• Conducting detailed clinical assessments to identify psychological, emotional, and social needs of patients
• Developing tailored treatment plans using evidence-based practices, including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT)
• Administering individual and family therapy sessions that focus on coping strategies, emotional support, and mental health education
• Documenting clinical information meticulously, adhering to HIPAA regulations and maintaining confidentiality
• Collaborating with an interdisciplinary treatment team to coordinate care with psychiatrists and other healthcare professionals
• Engaging in regular case discussions and consultations to refine therapeutic approaches and share clinical knowledge

Patient Population / Client Focus

This position specifically involves working with patients receiving hospice care as well as their families. You will provide vital emotional support and guidance to individuals navigating serious health challenges, often including terminal illnesses. Your empathetic approach will be crucial in helping clients find peace and closure while managing the psychological impact of their conditions.

Qualifications

• A Master's degree in Social Work from an accredited institution
• A valid state license as a Licensed Clinical Social Worker (LCSW)
• At least 2 years of clinical experience, especially focused on psychotherapy for adults
• Strong proficiency in solution-focused therapy techniques
• Excellent skills in communication, active listening, and establishing trust with clients
• Ability to work cohesively in a remote collaborative team environment

Preferred Experience

• Experience in grief counseling or palliative care is highly desirable
• Knowledge of community resources related to hospice care and mental health
• Familiarity with electronic health records (EHR) systems to streamline documentation

Compensation and Benefits
