Team meetings, case discussions, and collaborative planning will be facilitated through secure online platforms, ensuring continuity of care and shared clinical insights.

• Provide one-on-one psychotherapy sessions and mental health services to patients dealing with anxiety, depression, and post-traumatic stress disorder (PTSD), utilizing therapeutic techniques that promote healing and resilience.
• Conduct comprehensive assessments of patients’ mental health status, focusing on their emotional, behavioral, and social functioning to inform the development of tailored treatment plans.
• Develop, implement, and modify individualized treatment plans based on evidence-based practices, ensuring cultural competence and sensitivity to the unique backgrounds and needs of each patient.
• Collaborate closely with other healthcare professionals, including psychiatrists and primary care providers, to create a cohesive approach to patient care that addresses both physical and mental health needs.
• Monitor progress throughout the treatment process, adjusting strategies based on patient feedback and emerging clinical findings to foster optimal recovery.
• Educate and empower patients and their families about mental health conditions, treatment options, and available community resources to enhance their support network.
• Adhere to the highest ethical standards by maintaining patient confidentiality, obtaining informed consent, and ensuring compliance with all relevant regulations.

The LCSW will work primarily with hospice patients, supporting individuals in navigating the emotional challenges associated with terminal illness and end-of-life care. This role requires sensitivity to the complexities of grief and loss, alongside an understanding of how mental health conditions can affect individuals receiving hospice services.

• Master's degree in Social Work from an accredited institution.
• Current state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of 2-3 years of clinical experience providing psychotherapy and mental health services, particularly with vulnerable populations.
• Excellent verbal and written communication skills for effective patient interactions and collaboration with interdisciplinary teams.
• Proficient clinical skills, including comprehensive assessment techniques and the ability to develop robust treatment plans tailored to individual patient needs.
• Experience with electronic health records (EHRs) and related healthcare software systems to manage patient documents and treatment plans effectively.

• Familiarity with hospice care dynamics and the specific mental health needs of patients in palliative settings.
• Demonstrated success in working within interdisciplinary teams to enhance patient outcomes.

This position offers a competitive salary ranging from $90,000 to $105,000 annually, commensurate with experience and qualifications. Benefits include comprehensive medical, dental, and vision coverage, along with short-term and long-term disability options, life insurance, a 401(k) plan with contributions, and a flex spending account.
