• Conduct thorough assessments to identify clients' social, emotional, and environmental challenges critical for developing tailored intervention strategies.
• Create and implement individualized service plans, including specific goals, intervention strategies, and methods for progress evaluation.
• Provide empathetic and effective counseling to individuals and families facing a variety of life challenges, particularly in the realm of substance use recovery.
• Engage collaboratively with clients, guiding them in creating personalized service plans and connecting them to community resources that enhance their recovery process.
• Deliver crisis intervention and risk assessment services, providing immediate support and safety planning for clients in distress.
• Develop and maintain relationships with community organizations to coordinate comprehensive services and advocate for client needs.
• Participate in community outreach efforts aimed at increasing awareness of available services and promoting prevention strategies for substance abuse.
• Engage in regular case conferences, team meetings, and ongoing professional development through staff training sessions.

In this role, you will primarily work with individuals and families dealing with substance use disorders, mental health struggles, and associated psychosocial issues. You will be at the forefront of enabling clients to access critical support systems, thereby assisting them in their recovery journey and improving their overall quality of life.

• A Master's degree in Social Work from an accredited institution is required.
• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York.
• Strong understanding of evidence-based practices in the field of social work, particularly with populations experiencing substance use challenges.
• Demonstrated knowledge of community resources, trauma-informed care, and socio-economic justice issues.
• Exceptional communication skills, with an emphasis on cultural competence and problem-solving abilities.

• Experience working in a social service agency or similar environment, especially with populations struggling with addiction or mental health issues.
• Prior involvement in community outreach initiatives aimed at promoting social services and enhancing client engagement.
• Familiarity with electronic record-keeping systems and telehealth practices.
