• Conduct secure and confidential real-time therapy sessions with clients using our advanced telehealth technology.
• Perform comprehensive evaluations to accurately assess clients’ psychological, social, and emotional needs, enabling the formulation of personalized treatment plans.
• Provide timely crisis intervention and support to clients experiencing acute distress, ensuring their immediate safety and comfort.
• Facilitate client access to essential community resources and social services that promote their emotional and psychological well-being.
• Maintain detailed and confidential electronic client records, including progress notes and treatment plans, adhering strictly to legal and ethical standards.
• Collaborate regularly with interdisciplinary teams—including physicians, nurses, and other healthcare professionals—to ensure comprehensive and cohesive care for hospice patients.
• Deliver education on coping mechanisms, communication strategies, and self-care practices to enable clients to manage their mental health proactively.
• Engage in continuous professional development by staying informed about the latest best practices in social work and teletherapy methodologies.

This role is focused specifically on clients receiving hospice care, addressing their unique emotional and psychosocial challenges. You will work closely with patients and their families, providing empathetic support and guidance as they navigate complex feelings around mortality, loss, and grief. Your interventions will aid in easing anxiety and fostering a sense of peace during their end-of-life journey.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Arizona.
• A Master’s degree in Social Work (MSW) or a related discipline.
• A minimum of three years of clinical experience in social work, preferably with an end-of-life care focus.
• Proficiency in telehealth software and remote counseling practices.
• Exceptional communication and interpersonal skills, particularly in virtual platforms.
• Commitment to maintaining confidentiality and ethical treatment standards.
• Ability to independently manage a caseload in a remote teletherapy environment.
• Flexibility to accommodate diverse client schedules across different regions and time zones.

• Experience in hospice care or working with terminally ill patients is highly desirable.
• Familiarity with community resources relevant to mental health supports in end-of-life scenarios.
• Advanced training in therapeutic modalities that support grief counseling and bereavement.

This full-time position offers a competitive salary range of USD 80,000 – 100,000 annually, commensurate with your experience and qualifications. In addition to a robust salary, we offer:
