• Manage a personalized caseload of clients, ensuring to establish rapport while upholding professional ethics and boundaries.
• Conduct regular sessions with clients to assess their progress, discuss stability, and address any needs for discharge planning when appropriate.
• Develop and implement individualized treatment plans that focus on a client-centered approach, continually monitoring progress and making adjustments as necessary.
• Maintain accurate documentation and detailed progress notes for each client interaction in compliance with industry standards.
• Conduct thorough intake assessments to evaluate new clients thoroughly and ensure appropriate service matching.
• Provide individual therapy sessions weekly, demonstrating a commitment to client engagement and therapeutic effectiveness.
• Participate in supervisory sessions to discuss case management and professional development.
• Collaborate closely with multidisciplinary teams to provide holistic care, ensuring seamless integration of services.

The clients you will serve range from children to adults, encompassing various mental health challenges. • A Master’s degree in mental health counseling or a related field.
• Current licensing as a Licensed Clinical Professional Counselor (LCPC) with no restrictions on practice.
• Minimum of one year of post-graduate clinical experience in an outpatient setting providing direct psychotherapy services.
• Strong computer skills, including proficiency in Microsoft Excel, RMS systems, Outlook, and other therapeutic documentation platforms.
• Comfortable conducting detailed intake assessments and determining appropriate therapeutic interventions.

• Experience working in community health settings or similar outpatient environments.
• Familiarity with electronic health records (EHR) and case management software to facilitate efficient documentation and communication.
• Previous exposure to collaborative care models, integrating various healthcare provisions to enhance overall patient outcomes.

First Health Care LLC offers a competitive salary range of USD 75,000 – 90,000 annually, which is based on experience and qualifications.
