Key Responsibilities

• Conduct thorough psychosocial assessments through interviews, observations, and comprehensive reviews of medical records to identify emotional, social, and environmental factors that are influencing patient recovery.
• Provide empathetic counseling and emotional support to patients and their families, assisting them in navigating adjustment issues and improving overall well-being.
• Work collaboratively with a multidisciplinary rehabilitation team to create and implement individualized treatment plans that address psychosocial needs and enhance patient outcomes.
• Facilitate access to community resources, including support groups, vocational rehabilitation programs, and housing assistance to better equip patients post-rehabilitation.
• Offer educational support to patients and families regarding available community resources and aftercare services, ensuring they are well-informed and prepared for discharge.
• Participate actively in discharge planning, working to ensure smooth transitions from the detox center to home or alternate care settings.

Qualifications

• A Master’s degree in Social Work from an accredited program, ensuring strong foundational knowledge and skills.
• Active licensure as a Licensed Clinical Social Worker (LCSW) within the state of California, demonstrating your commitment to professional standards and ethics.
• Strong communication and interpersonal skills, essential for building rapport with patients, families, and colleagues.
• Sound theoretical knowledge of adjustment issues, grief, and coping mechanisms, which will empower you to support clients effectively.

Preferred Experience

• Experience working in medical or rehabilitation settings, which would enhance the depth of understanding for patients facing recovery journeys.
• Familiarity with advanced rehabilitation methodologies and interdisciplinary approaches that emphasize collaboration for patient-centered care.

Compensation and Benefits
