Mindful Steps Counseling Services is actively seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to deliver exceptional support and care to seniors within our hospice center. This full-time, remote position allows you to positively impact the lives of older adults by assessing their psychosocial needs and coordinating necessary services. This position is primarily remote, allowing you to manage your schedule and connect with clients from the comfort of your home. You will maintain a flexible work schedule, with core hours from Monday to Friday, 8:00 AM to 4:30 PM. While most work will be done off-site, occasional travel may be required for home visits or meetings with healthcare professionals and families. Your adaptability will be crucial in responding to clients' needs while balancing remote work dynamics.

• Conduct comprehensive assessments of clients' psychosocial needs by engaging in in-depth conversations and employing standardized assessment tools.
• Work collaboratively with clients and family members to formulate personalized care plans that align with individual goals and preferences, ensuring comprehensive support.
• Provide therapeutic counseling and emotional support, guiding seniors and their families through life transitions such as illness, loss, and end-of-life planning.
• Act as an advocate for clients, helping them understand their rights and access necessary community resources, including support groups and financial assistance programs.
• Partner with an interdisciplinary team of nurses, doctors, caregivers, and other professionals to ensure cohesive and integrated care delivery.
• Perform regular visits to assess client well-being, monitor living conditions, and address any potential safety concerns.
• Facilitate effective communication among family members, helping to resolve conflicts and promote a supportive caregiving environment.
• Educate clients and families about available resources and programs to encourage engagement and social participation.
• Maintain thorough documentation of assessments, care plans, and progress notes in compliance with regulatory requirements and best practices.
• Stay updated on social work policies, ethical standards, and current best practices through continuing education and professional development.

In this role, you will focus on supporting seniors, often dealing with complex psychosocial circumstances related to aging, illness, or loss. Our clients come from diverse backgrounds, each presenting unique needs that require tailored interventions. Your expertise will provide them with the empathy and guidance necessary to navigate this critical stage of their lives.

• A Master's degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• At least two years of experience in a social work role, preferably within a hospice, community center, or geriatric setting.
• Strong knowledge of social work principles, aging-related challenges, and relevant community resources.
• Excellent skills in assessment, counseling, empathy, and problem-solving.
• Proficiency in electronic health record (EHR) systems and computer applications.

• Prior experience working with the elderly, particularly in hospice or palliative care settings.
• Familiarity with healthcare regulations and compliance standards that govern social work practices.
• Experience collaborating with multi-disciplinary teams to deliver comprehensive care.
