Mindful Steps Counseling Services is actively seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to deliver exceptional support and care to seniors within our hospice center. This full-time, remote position allows you to positively impact the lives of older adults by assessing their psychosocial needs and coordinating necessary services. Work Environment

This position is primarily remote, allowing you to manage your schedule and connect with clients from the comfort of your home. You will maintain a flexible work schedule, with core hours from Monday to Friday, 8:00 AM to 4:30 PM. While most work will be done off-site, occasional travel may be required for home visits or meetings with healthcare professionals and families. Your adaptability will be crucial in responding to clients' needs while balancing remote work dynamics.

Key Responsibilities
  • Conduct comprehensive assessments of clients' psychosocial needs by engaging in in-depth conversations and employing standardized assessment tools.
  • Work collaboratively with clients and family members to formulate personalized care plans that align with individual goals and preferences, ensuring comprehensive support.
  • Provide therapeutic counseling and emotional support, guiding seniors and their families through life transitions such as illness, loss, and end-of-life planning.
