• Conduct thorough and ongoing assessments to determine the mental health needs of patients, utilizing comprehensive evaluation tools.
• Effectively communicate with a variety of stakeholders including supervisors, direct care staff, family members, and guardians, ensuring everyone is informed about an individual’s progress.
• Develop and provide a tentative schedule of therapy sessions, home visits, and observations, allowing for efficient monitoring of client progress.
• Coordinate and conduct home visits, facilitating essential observations, chart reviews, and staff interviews, which contribute to holistic data analysis for outcome improvements.
• Ensure that evidence-based program plans are implemented effectively and adhered to in practice, contributing to successful treatment outcomes.
• Participate actively in Interdisciplinary Team Meetings to discuss case progress and strategies, promoting collaborative care approaches.
• Prepare and complete monthly and quarterly progress notes, documenting changes in client conditions and treatment responses thoroughly.
• Organize and lead training for staff and team members to ensure competency in the program plan's implementation and documentation protocols.
• Conduct follow-up interviews and home visits to provide ongoing support, adapting interventions as necessary to meet the evolving needs of clients.
• Maintain meticulous records to ensure information sharing with relevant stakeholders is accurate and timely.
• Act as a liaison between the clinic, the public, and the broader mental health community, fostering partnerships that support developmental goals for our clients.
• Collaborate in creating procedures and training materials that enhance existing programs, ensuring continuous improvement in service delivery.

As a Licensed Professional Counselor at Oaks Integrated Health Centre, you will provide services to a diverse array of clients, including both adults and youth. This role demands cultural competency and sensitivity as you engage with individuals from varied backgrounds, each presenting unique challenges and needs. • Master's or Doctoral degree in Counseling, Psychology, or a related field is required.
• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Oregon.
• Minimum of two years of experience in a counseling role, with a preference for clinical experience in mental health settings.
• Knowledge of and experience in specializations such as family therapy, couples therapy, trauma-informed care, or substance misuse treatment.
• Strong grasp of legal and ethical standards governing mental health practice, ensuring compliance and best practices in service delivery.
• Exceptional interpersonal and communication skills, with a proven ability to establish rapport with clients and families.
• Demonstrated capability to work collaboratively within a healthcare team environment.
• Proficiency with Electronic Medical Records (EMR) software and related technologies to enhance patient care management.

• Specialized training or certifications in areas such as trauma-informed care or crisis intervention are highly regarded.
• Experience working in community health settings or non-profit organizations is advantageous.
• Familiarity with evidence-based treatment models and their application in clinical practice.
