Key Responsibilities
  • Conduct thorough ongoing assessments to evaluate patient needs and develop individualized treatment plans.
  • Facilitate communication among supervisors, team members, family, and other stakeholders regarding client progress and treatment outcomes.
  • Create and share a structured schedule with supervisors detailing appointments, home visits, and progress updates.
  • Coordinate and carry out home visits to perform in-home assessments, record reviews, and gather data essential for treatment efficacy.

  • Actively participate in Interdisciplinary Team Meetings to collaboratively discuss client cases and treatment approaches.

  • Facilitate training sessions to ensure all staff members are competent in implementing treatment programs and maintaining thorough documentation.
  • Provide ongoing follow-up interviews and home visits to continuously assess and support clients in their therapeutic journey.
  • Serve as a liaison to the public and mental health services community to aid in the achievement of development goals.
  • Assist in the development and enhancement of training materials and procedures to improve program effectiveness.
