This position is primarily remote, which allows for a flexible work-life balance while engaging in meaningful patient interactions. While you will be based remotely, you'll collaborate closely with our integrated healthcare team, including mental health professionals, primary care providers, and specialists at our Detox Center. Regular virtual meetings, case conferences, and supervision sessions facilitate a cohesive work environment that fosters professional support and development.

• Establish rapport quickly with patients and their families through effective communication methods tailored to individual preferences, ensuring clarity and understanding.
• Work collaboratively within a professional team of behavioral health and primary care providers, using your expertise to support comprehensive patient care.
• Conduct thorough and evidence-based screenings for mental health conditions including depression, anxiety, and substance abuse, while arranging for detailed assessments as necessary.
• Develop and maintain a diverse patient caseload, providing follow-up interventions that promote ongoing health and recovery.
• Utilize electronic medical records to document patient interactions and treatment plans within 48 hours of patient engagements to ensure continuity of care.
• Create integrated care plans that link patients to a variety of essential services, facilitating seamless information exchange among healthcare providers and the patient’s support network.
• Safeguard patient privacy and maintain strict confidentiality in all communications, records, and data management practices.
• Engage in ongoing supervision and participate in quality improvement meetings to enhance service delivery and uphold high standards of care.
• Refer patients requiring psychiatric medications to the consulting psychiatrist, ensuring that all treatments are well-coordinated.
• Identify and connect patients with relevant community resources—educational, vocational, and social—supporting their overall goals for well-being.
• Perform other duties as required to support the center and its operations effectively.

As an LCSW at Berks Community Health Center, you will work primarily with individuals facing challenges related to substance abuse and mental health disorders. Understanding the unique needs of our community, you’ll tailor your approach to engage with a diverse range of patients, including those from underserved backgrounds and those experiencing homelessness. Your ability to apply trauma-informed care principles will significantly impact their recovery journey.

• A Master's degree in a related Behavioral Health field is required.
• Current LCSW licensure is mandatory for this position.
• At least three years of experience in the behavioral health sector, with a focus on clinical social work.
• CPR or BLS certification is beneficial.
• Strong cultural competence and an understanding of the needs of the underserved populations we serve.
• Experience using a trauma-informed approach and understanding social justice frameworks.
• Solid knowledge of electronic medical record systems and the ability to use office management software competently.
• Bilingual in English and Spanish is preferred to enhance communication with our patient population.

• Experience working in integrated healthcare settings or substance abuse treatment facilities is beneficial.
• Knowledge of behavioral medicine and evidence-based treatments for comorbid conditions.
• Ability to conduct rapid assessments and manage crises effectively.
• Demonstrated skill in interdisciplinary collaboration.

This full-time LCSW position offers a competitive salary range of $90,000 to $100,000 annually. In addition to salary, we provide a comprehensive benefits package that includes:
