As a Licensed Clinical Social Worker at Carefree Calmness Counseling P.C., you will operate primarily in a remote work setting, optimizing convenience and flexibility while still collaborating closely with a supportive team. Our hospice center serves an array of clients in varying emotional and psychological states, allowing for a rich tapestry of experiences that contribute to professional development and enhanced client interaction.

• Conduct assessments and screenings to efficiently identify clients' needs, utilizing evidence-based practices to determine the appropriate level of care.
• Establish trusting relationships with clients, employing active listening and empathy to foster an environment conducive to healing.
• Develop, create, and implement individualized care plans that connect clients with ancillary services and support networks.
• Deliver focused therapy sessions—both one-on-one and in group settings—tailored to individual and familial needs.
• Collaborate with interdisciplinary teams and external agencies to ensure seamless client care and address potential barriers to treatment.
• Maintain accurate and timely documentation of client interactions, treatment plans, and progress for internal and regulatory compliance.
• Respond effectively to crises, intervening as necessary to ensure the safety and well-being of clients.
• Participate in ongoing professional development to enhance skills and knowledge, thereby improving service delivery.
• Carry caseloads that require regular monitoring of client progress, treatment adherence, and follow-ups to ensure continuity of care.
• Contribute to quality improvement initiatives by participating in committees focused on enhancing agency services.

You will work with a diverse patient population that includes individuals experiencing severe emotional disorders and significant mental health symptoms. This role is particularly focused on clients within our hospice facility, who may also be facing transitional or end-of-life challenges. Your work will help empower these clients and their families, ensuring collaborative support tailored to their unique circumstances.

• A Bachelor's Degree in Social Work or a related field is required; a Master's degree is preferred.
• A valid LCSW license in the State of Colorado.
• Minimum of 2 years of experience in clinical social work, particularly in mental health and hospice settings.
• Proficient understanding of social work principles, ethics, and care delivery frameworks.
• Exceptional interpersonal skills with the ability to work effectively across varied sociocultural contexts.
• Proven ability to collaborate with multidisciplinary teams and stakeholders.

• Experience in hospice or palliative care environments.
• Knowledge of trauma-informed care and family systems therapy.
• Familiarity with electronic health record (EHR) systems and data entry processes.
• Proficiency in crisis intervention techniques and conflict resolution.
• Desire and capability to engage in outreach activities to promote mental health awareness.

This position offers a competitive salary range of USD 70,000 to 85,000 annually, commensurate with experience and qualifications.
