• Conduct comprehensive clinical assessments to understand patients' psychosocial needs, helping to identify the necessary interventions tailored to their unique situations.
• Create and implement individualized treatment plans that utilize evidence-based and solution-focused therapeutic methods to promote mental wellness.
• Document patient interactions, interventions, and progress in an electronic health record system, ensuring regulatory compliance and high-quality data integrity.
• Facilitate care coordination with interdisciplinary teams, including physicians, nurses, and other healthcare providers, to ensure holistic treatment approaches.
• Participate in peer review meetings and case consultations, sharing insights and learning from colleagues to perpetuate professional development and enhance patient care.
• Educate patients and families on mental health resources and coping strategies, empowering them to make informed decisions and actively participate in their care plans.
• Stay informed on the latest research and developments in social work and mental health to ensure the highest standard of care.

You will work with a diverse clientele, primarily focusing on individuals facing terminal illnesses and their families. Understanding the emotional and psychological complexities associated with end-of-life care is integral to this role. Your empathetic approach will guide patients and their families through their fears, grief, and decision-making processes, ultimately enhancing their quality of life during a sensitive time.

• A Master’s degree in Social Work from an accredited institution.
• A valid LCSW license in New York State.
• A minimum of three years of clinical experience in social work, with expertise in psychotherapy and hospice care.
• Strong proficiency in evidence-based therapeutic interventions, particularly those that are solution-focused.
• Excellent verbal and written communication skills, along with strong problem-solving abilities and a collaborative mindset.

• Previous experience in hospice or palliative care settings is highly desirable.
• Familiarity with telehealth platforms and virtual counseling methods.

• Annual salary in the range of USD $80,000 – $100,000, commensurate with experience.
