Job Title: Quality Coder/Abstractor Location: Waynesboro Family Medicine and Obstetrics Position Type: Full-Time
Position Summary: We are seeking a detail-oriented and experienced Quality Coder/Abstractor to join our team. This role is vital in ensuring accurate coding, documentation integrity, and the promotion of patient wellness. The ideal candidate will review patient encounters for appropriate Hierarchical Condition Categories (HCC) diagnosis codes, Level II HCPCS codes, ensure compliance with documentation standards and submit quality data through appropriate payer portals . Additionally, this position will support efforts to maintain preventive care by monitoring wellness exam schedules and coordinating patient appointments.
Key Responsibilities:
· Review and abstract patient encounters for accurate and compliant HCC diagnosis codes, Level II HCPCS codes, and supporting documentation.
· Identify and address discrepancies in coding and documentation to maintain high standards of accuracy and compliance.
· Ensure the integrity of patient records by verifying that all information is appropriately documented and updated.
· Monitor and ensure patients are scheduled for preventive wellness exams, proactively identifying any gaps in care.
· Schedule and coordinate wellness appointments with patients, maintaining a smooth workflow within the physician’s office.
· Stay updated on coding guidelines, compliance regulations, and any changes in healthcare coding standards.
· Collaborate with healthcare providers and administrative staff to support proper documentation and efficient patient care workflows.
Qualifications:
· Certification in medical coding (e.g., CPC, CCS, or equivalent) is preferred.
· Strong knowledge of HCC coding, Level II HCPCS coding, and healthcare documentation standards.
· Attention to detail and ability to review large volumes of data accurately.
· Excellent organizational and time-management skills.
· Strong communication skills and the ability to collaborate with a multidisciplinary team.
· Proficiency in healthcare coding software and electronic health records (EHR) systems is preferre
