This role is primarily remote, allowing you the flexibility to provide care from your own workspace while remaining a pivotal part of our hospice center. As a remote LCSW, you will leverage telehealth technology to conduct assessments, counseling sessions, and follow-up care efficiently.

As a Licensed Clinical Social Worker, your day-to-day responsibilities will include, but are not limited to:

• Conducting comprehensive biopsychosocial assessments tailored to the unique needs of patients in hospice care.
• Documenting mental health evaluations and individual and group counseling sessions electronically to maintain accurate patient records.
• Facilitating crisis interventions, developing treatment plans, and making referrals to appropriate community resources and services.
• Providing compassionate counseling and emotional support for patients, families, and caregivers facing end-of-life decisions, while exploring coping strategies and therapeutic options.
• Collaborating with healthcare providers to escalate patient needs and coordinate care for complex cases, ensuring a holistic approach to patient wellness.
• Participating in Quality Improvement initiatives to enhance service delivery and address emerging patient needs through data collection and analysis.

You will primarily work with patients facing terminal illnesses and their families within a hospice care framework. This population requires specialized care that addresses not only the medical aspects of their condition but also the intense emotional and psychological challenges of end-of-life scenarios. Your role is crucial in providing support that fosters dignity, comfort, and quality of life during this sensitive time.

The ideal candidate must possess the following qualifications:

• A Master’s degree in Social Work from an accredited program and active licensure as a Licensed Clinical Social Worker (LCSW) in Pennsylvania.
• A minimum of three (3) years of post-licensure experience, preferably within a behavioral health or clinical setting.
• Demonstrated knowledge of counseling techniques, crisis intervention strategies, and the ability to exercise independent judgment in a clinical context.
• Familiarity with telehealth technologies and electronic documentation systems is highly desirable.

While not mandatory, candidates with the following experience will be given preference:

• Two or more years of experience specifically in hospice or palliative care settings.
• Skills in group facilitation and program development.
• A track record of building strong relationships with community resources and agencies for effective patient referrals.
