Key Responsibilities
  • Conduct comprehensive assessments to identify the needs of your patients, developing therapeutic plans tailored to their unique situations.
  • Maintain clear and ongoing communication with supervisors, family members, and other stakeholders regarding patient progress and treatment outcomes.
  • Provide supervisors with a tentative schedule for appointments and home visits, ensuring efficient management of your caseload.
  • Coordinate and conduct home visits as needed to support in-home observations, data collection, and chart reviews.
  • Implement and monitor the effectiveness of treatment plans, adjusting strategies as necessary to align with each patient’s evolving needs.
  • Actively participate in interdisciplinary team meetings to discuss patient progress and collaborate on care strategies.
