As a Licensed Mental Health Counselor, you will engage in a variety of tasks that are crucial to the treatment and support of our clients. Your responsibilities will include:

• Communicating effectively with physicians and other health care team members to report on patient progress, needs, and concerns.
• Consulting and coordinating with clients' family members or support systems regarding treatment planning and discharge readiness.
• Maintaining accurate and compliant clinical documentation, including progress and clinical notes, in accordance with department policies.
• Collaborating with the health care team to develop individualized treatment and discharge plans based on comprehensive psychosocial assessments.
• Facilitating discharge planning and care coordination, ensuring clients have access to needed services and resources.

Your work will involve supporting a diverse patient population, including those facing various mental health challenges such as depression, anxiety, PTSD, and substance abuse disorders. By employing a client-centered approach, you will empower individuals to navigate their mental health journeys and achieve sustainable recovery outcomes.

The ideal candidate for this role will possess:

• A Master's degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology.
• An active state licensure as an LCSW, LMFT, LPC, or an equivalent credential.
• A minimum of 3 years of professional experience in an outpatient mental health setting, demonstrating a strong capacity to address complex clinical issues.
• Proficiency in using digital tools, including electronic medical records, word processing applications, and communication platforms.
• The ability to work autonomously, demonstrating strong organizational and time management skills.

In addition to the qualifications outlined above, candidates with experience in group therapy facilitation, substance use treatment, or evidence-based therapeutic modalities will be given priority. Understanding the nuances of telehealth delivery and strong interpersonal skills will also enhance your effectiveness in this role.
