This role offers a remote work setting, allowing for flexibility while maintaining a strong connection with the community we serve. You will have the opportunity to engage with clients via phone and video consultations. We also acknowledge that client needs may require occasional evening or weekend hours, hence some flexibility in your schedule will be required. • Conduct comprehensive assessments of seniors' psychosocial needs, accounting for emotional, mental, and social factors impacting their well-being.
• Work closely with clients and their families to design individualized care plans tailored to their unique goals, preferences, and circumstances.
• Provide counseling and emotional support, assisting clients and their families in navigating challenging life transitions.
• Act as an advocate for clients, ensuring they can access necessary community resources and services.
• Collaborate with interdisciplinary teams comprising nurses, healthcare professionals, and caregivers to coordinate effective care delivery.
• Regularly visit clients to monitor their well-being, assess living conditions, and address any safety or emotional concerns.
• Facilitate communication and resolve conflicts among family members to promote effective teamwork in the care process.
• Educate clients and families about available community resources, support groups, and programs to encourage social engagement.
• Maintain accurate client documentation, including assessments, care plans, and interaction notes, ensuring compliance with ethical standards and state regulations.
• Stay informed about social work best practices and relevant regulations to provide the highest standard of care.

This role primarily focuses on the senior population, addressing their diverse psychosocial needs. The experience requires you to engage empathetically with elderly clients and their families, understanding the complexities that come with aging. Your work is instrumental in helping clients manage transitions, such as moving into assisted living or confronting significant health challenges, and your insights can lead to improved outcomes and quality of life for those we serve.

• A Master's degree in Social Work from an accredited institution is required.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work role, with a preference for experience in a community center or geriatric environment.
• Strong knowledge of social work principles, aging issues, and available community resources.
• Excellent skills in assessment, counseling, and problem-solving.
• Ability to empathize and establish trust with seniors and their families.
• Strong communication and interpersonal skills are critical for effective teamwork.
• Proficiency in electronic health record (EHR) systems and digital communication tools.

• Experience with chronic illness management and palliative care, particularly within hospice settings.
• Familiarity with local resources and programs available to seniors in Tennessee.
• Demonstrated ability to work in a remote setting successfully while maintaining strong connections with clients and team members.
