Compensation
Salary Range: $90,000 - $105,000 per year
Bonus / Commission (if applicable):
This role is eligible for a $15,000 sign-on bonus.
Location: Remote (Hybrid training required: 2 days onsite / 3 days remote for 4-6 months)

Position Summary
As an RN Care Manager within an MLTC program, you will conduct telephonic assessments and develop person-centered service plans to support members' medical, psychosocial, and community-based needs. This role is focused on care coordination, ensuring continuity across healthcare settings, and improving quality of life through education and intervention. You will collaborate with interdisciplinary teams, providers, and caregivers while ensuring compliance with CMS and DOH regulations.
Responsibilities

  • Shift details: Monday - Friday (Business Hours)
  • Hours per week: 40

? Key Duties:
  • Conduct telephonic assessments and review UAS-NY data to develop Person Centered Service Plans (PCSP)
  • Coordinate care by collaborating with PCPs, specialists, and interdisciplinary teams
  • Assist members in accessing necessary medical, social, and community-based services
  • Identify care gaps and educate members on preventative health and disease management
  • Ensure accurate and timely documentation of care plans in compliance with regulatory standards

Requirements
? Education
  • Required: Associate's Degree in Nursing
  • Preferred (if applicable): Bachelor of Science in Nursing (BSN)

? Licenses & Certifications
  • Required: Active New York State Registered Nurse (RN) license

? Skills:
  • Strong knowledge of Medicare, Medicaid, and MLTC regulations
  • Excellent telephonic assessment, care coordination, and documentation skills
  • Strong communication and collaboration abilities
  • Bilingual Spanish preferred
