The Brookdale Counseling Center is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to join our team as a full-time clinician specializing in hospice care. • Engage directly with clients experiencing major mental illnesses and emotional disorders, focusing on culturally competent care that respects individual beliefs and practices.
• Conduct thorough assessments and screenings to identify clients' needs and determine appropriate treatment plans, using evidence-based practices.
• Create individualized integrated care plans that link clients to vital services, coordinating with family members and other healthcare providers to ensure comprehensive support.
• Deliver a range of therapeutic services, including individual, group, and family therapy, tailored to clients' specific presenting issues.
• Assist in crisis intervention when necessary, utilizing established methodologies to ensure client safety and well-being.
• Monitor client progress through regular follow-ups, ensuring that treatment aligns with their evolving needs and adjusting care plans accordingly.
• Participate in interdisciplinary team meetings to share insights and collaborate on patient care strategies.
• Maintain accurate documentation, including treatment notes, progress reports, and data submissions, adhering to ethical standards and confidentiality protocols.
• Support quality improvement initiatives by actively participating in committees focused on enhancing service delivery.
• Provide on-call coverage as needed, ensuring continuity of care for clients outside of regular hours.

The LCSW will primarily work with clients in hospice care, addressing a diverse range of mental health symptoms arising from terminal illnesses and profound life transitions. This role requires a deep understanding of the emotional and psychological complexities faced by patients and their families during a critical stage of life. The caregiver will adopt a holistic approach to treatment, emphasizing empowerment, dignity, and respect for each client's unique journey.

• A valid Texas license as a Clinical Social Worker (LCSW) is required.
• A Bachelor's degree in Social Work; a Master's degree is preferred and may enhance the depth of service provided.
• A minimum of 2 years of clinical experience in social work, particularly with hospice or palliative care populations.
• Proficiency in conducting assessments and implementing treatment plans in a culturally aware manner is essential.

• Experience working with clients facing severe emotional disturbances related to end-of-life issues.
• Familiarity with community resources available for hospice clients and their families.
• A proven track record of providing crisis intervention and short-term support services.

This full-time position offers competitive compensation with a salary range of USD 70,000 to 85,000 annually, commensurate with experience.
