• Establish and maintain rapport with patients, families, and the treatment team by employing active listening techniques and communicating clearly in a supportive, non-judgmental manner.
• Conduct comprehensive screenings and assessments for mental health conditions, including depression, anxiety, and substance use disorders, and facilitate access to appropriate treatment resources.
• Manage a patient caseload, delivering follow-up interventions and monitoring progress toward individual care goals.
• Utilize the electronic medical record system to streamline documentation and improve the integration of healthcare services, ensuring that records are complete within 48 hours of each patient encounter.
• Develop and implement personalized care plans that address the unique needs of each patient, ensuring effective coordination with all members of the treatment team.
• Safeguard patient confidentiality and adhere to all ethical guidelines and regulatory requirements in documentation, communications, and care delivery.
• Participate in regular supervision, case conferences, and quality improvement meetings to contribute to team development and organizational effectiveness.
• Maintain communication with patients and their families, returning calls promptly to address concerns or schedule appointments.
• Promote community resource linkage to support patients' social, educational, and vocational needs in line with their health goals.
• Other relevant duties as assigned to enhance service delivery and patient outcomes.

This role focuses on providing care to underserved populations in the community, including individuals from low-income backgrounds, those experiencing homelessness, and others facing barriers to accessing mental health services. You will be instrumental in helping these individuals navigate the complexities of behavioral health within the broader context of their medical care.

• A Master’s degree in a related behavioral health field is required.
• Active LCSW licensure in Pennsylvania.
• Current CPR or BLS certification.
• A minimum of 3 years of experience working in the behavioral health field.
• Strong cultural competence and understanding of the unique needs of underserved populations.
• Experience with trauma-informed care principles and social justice frameworks.
• Proficiency in office management, computer software applications, and electronic health record systems.

• Bilingual proficiency in English and Spanish is preferred to enhance communication with our diverse patient base.
• Experience working within a primary care behavioral health integration model.
• Demonstrated ability to work effectively within interdisciplinary teams and respond to emergency situations with composure.
• Strong problem-solving skills and attention to detail to ensure high-quality patient care.
