You will work within an outpatient facility model, engaging with patients through virtual consultations and a range of digital platforms to ensure accessibility of care.

• Conduct comprehensive mental health assessments to evaluate the specific needs and challenges of elderly patients, addressing factors such as cognitive function, emotional health, and social support systems.
• Develop, implement, and monitor individualized treatment plans that incorporate evidence-based practices and consider each patient’s cognitive, physical, and emotional health.
• Utilize a range of therapeutic interventions, including individual and family therapy, to provide holistic support for psychiatric conditions prevalent in older adults.
• Collaborate closely with geriatric psychiatrists and a multidisciplinary team to ensure cohesive care delivery, integrating various healthcare perspectives for optimal patient outcomes.
• Deliver crisis intervention services as needed, providing immediate support during mental health emergencies.
• Educate patients and their families about mental health conditions, treatment plans, and available community resources to foster understanding and empower informed decision-making.
• Maintain accurate and confidential documentation of patient information and treatment approaches, complying with legal and ethical standards of practice.
• Participate in regular team meetings, case consultations, and professional training activities to continuously enhance your clinical skills and stay updated on advancements in geriatric mental health.

Your primary focus will be the geriatric population, characterized by diverse mental health needs arising from aging processes and life transitions. You will be equipped to address common issues such as depression, anxiety, cognitive decline, and adjustment disorders, all while considering the individual circumstances and cultural backgrounds of elderly clients.

• A Master’s degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.
• Current and valid licensure as an LCSW, LPCC, or LMFT in the state of California.
• A minimum of two (2) years of post-licensure clinical experience is preferred.
• Strong understanding of geriatric mental health disorders and proficiency in evidence-based treatment modalities.
• Experience providing therapy to individuals and families within an older adult community is highly valued.
• Exceptional interpersonal and communication skills, demonstrating a patient-centered and empathetic approach to care.
• Ability to build rapport and trust with clients, their families, and healthcare teams.
• Competency in utilizing electronic health record (EHR) systems and proficiency in various computer applications.
• A collaborative mindset to effectively contribute within a multidisciplinary team setting.

• Experience working in a geriatric healthcare setting or mental health practice specializing in senior care.
• Familiarity with community resources and support systems beneficial for geriatric patients.
