This position is primarily remote, offering the flexibility to provide care to patients from the comfort of your own space while being supported by a dynamic outpatient office team. You will work Monday to Friday from 8:00 AM to 5:00 PM, with occasional evening shifts based on patient requirements. • Conduct thorough mental health assessments that take into account how medical conditions impact patients’ psychological well-being.
• Design and implement integrated treatment plans that cater to both psychiatric and medical needs, working collaboratively with both mental health professionals and medical providers.
• Facilitate individual and group therapy sessions, employing evidence-based therapies tailored to each patient's unique circumstances.
• Respond to mental health crises, providing immediate intervention and support during emergencies.
• Educate patients and their families about the interplay between mental and physical health, empowering them to manage their conditions effectively.
• Maintain accurate, confidential patient documentation in compliance with relevant laws and ethical standards.
• Engage actively in interdisciplinary team meetings, case discussions, and care planning to ensure coordinated patient care.
• Stay current with the latest research, therapeutic modalities, and best practices in integrated care.

You will have the opportunity to work with a diverse patient demographic at our outpatient office, particularly those who experience co-occurring mental health and medical conditions. This includes individuals managing chronic illnesses, mental health disorders, and substance use issues. Your role will be crucial in addressing these complex needs, providing a layer of support that encourages holistic well-being.

• A Master's or Doctoral degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or Psychology from an accredited institution.
• Current licensure as an LCSW, LCPC, or LMFT within the state of Illinois.
• Proven competency in addressing the mental health challenges faced by individuals with concurrent medical conditions.
• Ability to collaborate effectively with medical professionals in an integrated care setting.
• Exceptional verbal and written communication skills.
• Strong adherence to ethical standards, maintaining professional boundaries and patient confidentiality.
• Familiarity with electronic health record (EHR) systems and related computer applications.
• Proven ability to work collaboratively within an interdisciplinary team.

• At least 2 years of counseling experience, particularly in settings that emphasize integrated care.
• A background in managing or working with patients who have co-occurring medical and psychiatric issues is highly desirable.
