Work Environment

Key Responsibilities
  • Conduct comprehensive assessments to identify the emotional, mental, and social needs of seniors, establishing personalized care plans to meet their individual circumstances.
  • Engage in counseling sessions with seniors and their families, providing emotional support that empowers them to progress through life transitions, such as health changes and bereavement.
  • Advocate for clients' rights, acting as a liaison to facilitate access to community resources, including healthcare, social services, and support groups.
  • Maintain regular follow-ups through check-in sessions, ensuring the monitoring of clients’ well-being and adapting care plans as necessary based on their evolving needs.
  • Work collaboratively with a multi-disciplinary team of healthcare professionals, including nurses, caregivers, and mental health specialists, to coordinate comprehensive care that aligns with clients’ best interests.
  • Educate clients about available resources and services that can enhance their independence and social engagement, thus enriching their overall quality of life.
  • Document all client interactions accurately in electronic health record (EHR) systems, ensuring compliance with legal and ethical standards.
  • Participate in team meetings to discuss cases, share insights, and develop strategies that drive better client outcomes.
Patient Population / Client Focus

Your primary focus will be the senior community within Jacksonville and surrounding areas, addressing their unique challenges and helping them manage life transitions effectively. You will work with clients from diverse backgrounds, ensuring culturally sensitive practices in your interventions.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required.
  • A valid and current state license (LMSW) for the practice of social work in Florida.
  • At least two years of relevant experience in social work, ideally within a community health setting or geriatric care environment.
  • Robust understanding of aging-related issues and the principles governing effective social work practices.
  • Exceptional skills in assessment, counseling, and problem-solving, established through practical experience.
  • Ability to build rapport with seniors and their families, demonstrating empathy and patience during challenging times.
  • Strong verbal and written communication skills for effective collaboration and documentation.
  • Proficiency in electronic health record systems and basic computer applications.
Preferred Experience
  • Previous experience providing counseling in a remote capacity or using telehealth platforms.
  • Familiarity with community resources available to seniors and knowledge of relevant legal and ethical considerations in social work.
  • Experience working with interdisciplinary teams to improve client outcomes.
