Key Responsibilities
  • Engage directly with clients in therapy sessions, addressing significant mental health issues both in virtual appointments and, when necessary, in community outreach initiatives.
  • Collaborate with multidisciplinary teams to establish effective treatment plans tailored to each client's unique needs, facilitating the highest level of care and support.
  • Conduct thorough assessments and evidence-based screenings to identify the specific mental health needs of patients.
  • Develop, implement, and adjust individualized care plans while ensuring continuous communication with clients and their families regarding treatment progress.
  • Participate actively in team meetings, sharing insights and strategies that enhance service delivery, as well as aligning team objectives with overall organizational goals.
  • Identify risks and implement safety measures to create a secure environment for both clients and staff.
  • Provide crisis intervention services to clients in urgent need, ensuring timely and appropriate therapeutic responses.
  • Remain compliant with industry standards, ethical guidelines, and the internal policies set forth by Community Outreach Mental Care.
Patient Population / Client Focus

The LCSW will serve a diverse client base, primarily consisting of individuals from underserved communities facing a range of mental health challenges, including anxiety disorders, depression, trauma, and other serious mental illnesses. Qualifications
