This role is exclusively remote, allowing for flexibility while ensuring you can maintain close contact with clients through virtual consultations. The full-time work schedule is Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evenings or weekends required based on client needs. On-call availability may also be necessary to provide instant support in urgent situations.

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting thorough assessments of seniors' psychosocial needs, considering their emotional, mental, and social factors.
• Developing personalized care plans in collaboration with clients and their families to address specific goals and needs.
• Providing counseling and emotional support to help seniors navigate life transitions, such as health changes or loss of loved ones.
• Advocating for clients' rights and facilitating their access to essential community resources to ensure they receive appropriate services.
• Coordinating care with interdisciplinary teams, enhancing the collaborative approach to client health and well-being.
• Regularly monitoring clients' well-being through virtual visits to assess living conditions and address potential safety concerns.
• Educating clients and families about available support groups and community programs to foster social engagement.
• Maintaining accurate documentation of client interactions, assessments, and care plans in compliance with ethical and regulatory standards.

Your primary focus will be on supporting seniors, a population that often faces complex physical and emotional challenges as they navigate aging. • Master's Degree in Social Work from an accredited institution.
• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.
• Minimum of two years of experience in a social work position, ideally within a community health clinic or geriatric setting.
• Demonstrated knowledge of social work principles, aging-related issues, and existing community resources.
• Exceptional assessment, counseling, and problem-solving skills.
