Position Overview

Community Health Connection is seeking a dedicated and compassionate Clinical Licensed Marriage and Family Therapist (LMFT) to provide remote counseling services to individuals, couples, and families. Key Responsibilities
  • Build and maintain a trusting and therapeutic relationship with patients to enhance engagement and treatment adherence.
  • Conduct comprehensive assessments to diagnose mental health disorders and develop personalized treatment plans tailored to each patientโ€™s unique circumstances.
  • Provide regular counseling sessions to individuals and couples, adapting therapeutic techniques to meet evolving needs.
  • Utilize evidence-based practices to deliver effective therapy and monitor patient progress through ongoing assessments and adjustments to treatment plans.
  • Facilitate group therapy sessions that focus on improving communication, problem-solving, and coping skills within family dynamics.
  • Collaborate with interdisciplinary teams to ensure comprehensive care, including making referrals to additional specialists when necessary.
  • Document patient interactions, assessments, and treatment strategies accurately in compliance with organizational standards and confidentiality regulations.
Patient Population / Client Focus

In this role, you will primarily work with a diverse range of patients experiencing mental health challenges, relationship issues, and family dynamics. Your patients may include individuals coping with anxiety, depression, trauma, or substance use disorders. Your ability to establish rapport and provide empathetic care will be critical in guiding them toward recovery and healthier lifestyle choices.

Qualifications
  • A valid and unrestricted LMFT license in the state of Illinois.
  • A Master's or Doctoral degree in Marriage and Family Therapy, Counseling, Social Work, Psychology, or a related field.
  • Strong therapeutic skills with a demonstrated ability to motivate clients and facilitate positive change.

Compensation and Benefits
